    BACKGROUND TO THE MEDICAL PROGRAM

    Zanzibar, like many regions in Africa, faces a critical shortage of healthcare workers. Contributing factors include low wages, demanding working conditions, and limited access to professional training. These challenges are intensified by the ongoing demands of HIV/AIDS treatment, which place additional strain on an already under-resourced healthcare system.

    The most common causes of death in Zanzibar include malaria, HIV/AIDS, respiratory illnesses, and cardiovascular disease. While the healthcare system focuses heavily on urgent and acute care, there is also a strong emphasis on patient education and prevention to reduce long-term health risks.

    Medical volunteers are placed in government hospitals and community clinics in areas such as Paje and Stone Town, offering exposure to healthcare delivery in both urban and community-based settings. This variety provides a well-rounded understanding of the local medical landscape.

    This program is well suited to individuals seeking practical volunteer experience to strengthen future medical or healthcare careers. Participants may observe and assist across multiple departments, gaining broad and valuable exposure. Beyond the placement itself, volunteers also have the opportunity to experience Zanzibar’s rich culture and natural beauty.

    MEDICAL VOLUNTEER PLACEMENT EXAMPLES

    Mnazi Mmoja Hospital - Located in Stone Town, approximately 40 minutes from Paje, Mnazi Mmoja Hospital is the main government referral and teaching hospital in Zanzibar. It serves the wider island population and provides a range of services from general medicine to specialist care. The hospital is often busy and under-resourced, offering volunteers an authentic insight into public healthcare delivery.

    Mwembeladu Maternity Hospital - A partner facility to Mnazi Mmoja Hospital, this maternity hospital handles approximately 400 births each month. It is a particularly suitable placement for midwifery students seeking exposure to antenatal care, labour, and delivery in a high-demand environment.

    Volunteers may observe or rotate through departments such as General Medicine, Dental, Midwifery, Newborn Care, Paediatrics, Pharmacy, Surgery, Laboratory, Radiology, HIV Testing, Diabetes Clinic, Tuberculosis, Physiotherapy (part-time), and more. While preferences can be shared, placements are subject to availability.

    On the first day, volunteers receive an introduction from the hospital matron, who discusses interests and helps coordinate department rotations. It is recommended to spend at least one week in each department to gain meaningful exposure.

    This placement allows volunteers to encounter medical cases that are uncommon in many countries, including tuberculosis, typhoid, and malaria. Working in a government hospital can be challenging due to limited equipment and resources, and healthcare delivery may feel slower or less personal. Adapting to these conditions is often one of the most valuable learning aspects of the experience.

    STUDENT ELECTIVE OPPORTUNITIES

    Elective placements are available for students studying medicine, midwifery, or nursing. These electives are observational in nature and allow students to learn directly from local healthcare professionals. Requests for specific electives can be made during the application process.

    Medical Elective - Medical students are placed in government hospitals in Stone Town or Paje, where they shadow doctors and nurses during ward rounds and observe the diagnosis and treatment of common illnesses such as malaria and typhoid.

    Midwifery Elective - Midwifery students work alongside experienced midwives in maternity hospitals, observing antenatal clinics, labour, and childbirth. This placement highlights the realities and challenges of maternal care in resource-limited settings.

    Nursing Elective - Nursing students shadow nurses in government hospitals, assisting with patient care, monitoring vital signs, and administering medication under supervision. Opportunities may include community outreach activities such as health checks and screenings.

    Each elective offers valuable insight, skill development, and the chance to support essential healthcare services in Zanzibar.

    OTHER THINGS TO CONSIDER AS A MEDICAL VOLUNTEER IN TANZANIA

    Airport Transfers - Return airport transfers are not included. Local staff can assist with arranging onward transport once you are in-country, offering flexibility and the possibility of sharing costs with other participants. Public transport and tuk-tuks are not recommended for airport transfers due to safety and convenience concerns.

    Hands-On Involvement - Levels of practical involvement depend on medical background, program length, and staff discretion. Some participants have assisted with basic tasks such as wound dressing or IV preparation, but hands-on experience is not guaranteed. Volunteers without medical training typically take observational roles.

    Hospital Environment - Healthcare students who can provide proof of study may access additional learning opportunities. Volunteers should be prepared for cultural differences in patient care and emotional expression. Government hospital settings can be demanding, and volunteers are encouraged to step back and seek support if needed.

    Not for Qualified Professionals - This program is designed for school leavers and students, not for fully trained professionals seeking to practise medicine abroad.

    Participants Aged 17 - While open to participants aged 17 and above, younger volunteers should be confident and self-motivated. Large hospital environments can be overwhelming. Those seeking more structure may prefer alternative destinations.

    Multiple Placements - In most cases, volunteers are placed at a single facility. Combining multiple placements may be possible but requires an additional local donation.

    Yellow Fever Vaccination - Yellow Fever vaccination is legally required for travellers arriving from or transiting through high-risk countries. However, proof has sometimes been requested regardless. To avoid complications, obtaining the vaccination before travel is strongly recommended.

    Mandatory Zanzibar Insurance - All visitors to Zanzibar must purchase local insurance through the Zanzibar Insurance Corporation (ZIC) before arrival. Failure to do so may result in denied entry.

    Weekends - Project work runs Monday to Friday, leaving weekends free for rest or travel. Many volunteers use this time to explore Zanzibar’s beaches, culture, and nearby attractions.

If you have little or no prior medical experience, your role will mainly be observational, allowing you to gain valuable insight into the local healthcare system. Volunteers who are studying a medical or health-related degree may have limited opportunities for basic hands-on involvement, depending on their experience level, the trust built with local doctors and nurses, and patient consent.

    Your primary focus as a medical volunteer is to observe and understand healthcare practices in a developing country. This experience offers a deeper awareness of common health challenges on the island, such as malaria and typhoid, and how these conditions are managed in a resource-limited environment. It also provides a useful point of comparison between healthcare practices in Zanzibar and those in your home country.

    Volunteers typically spend between 3 and 5 hours per day at the placement, working Monday to Friday. Usual working hours are either 8:30am to 1:00pm or 11:00am to 4:00pm. Volunteers are often welcome to stay longer if they wish to gain further exposure and immerse themselves more fully in the experience.

    Transport to the medical placement is not included in the program fee. Most volunteers use local transport, such as a Dala Dala or Tuk Tuk, to reach their placement, with travel times of up to 40 minutes. During the in-country orientation, the local team will explain how the transport system works and provide guidance to help you feel confident using it. Support is always available if you have any questions or concerns regarding travel or pricing. This experience offers both meaningful learning and personal growth in a challenging and rewarding environment.

Leisure time can be a chance to forge friendships, rest between shifts, and make the most of Zanzibar’s unique island setting.

    For your volunteer trip to Zanzibar, please book your flights to arrive at Abeid Amani Karume International Airport (airport code ZNZ) on the designated Sunday start date. This is the official arrival day for the program and helps ensure a smooth and well-organised start to your experience as a medical volunteer in Zanzibar.

    Alojamiento

    During your time on the Zanzibar volunteer program, you will stay at the Volunteer House, located in Paje, a growing coastal town on the west coast of Zanzibar. This area is also where the volunteer projects are based. The house is set in a quiet neighbourhood, while restaurants, supermarkets, and ATMs are all within easy walking distance.

    You will share the Volunteer House with other international volunteers. The accommodation is spacious, simple, and comfortable. Free Wi-Fi is available, although the connection can be unreliable at times. Volunteers receive a local SIM card for use with an unlocked phone. The house also offers shared communal areas to relax in the evenings, a refrigerator for storing food and drinks, and access to filtered drinking water. A laundry service is available near the supermarket, or volunteers may choose to wash clothes by hand.

    For security, a night guard is on duty at the Volunteer House. Volunteers are advised to bring a small padlock for personal belongings, and valuables can be stored securely with the local team. It is recommended to bring only essential items. Please note that power cuts are common in Zanzibar but are usually short and last only a few hours.

    The atmosphere is friendly and welcoming, making it easy to connect with other volunteers. Most projects are within walking distance of the accommodation, while some require a short drive. Transport options will be explained by the local team during the orientation session.

    Comida y bebida

    Volunteers are provided with breakfast and dinner, freshly prepared on-site by a local cook. Meals are a combination of local and Western-style dishes. Lunch is not included, giving you the flexibility to visit nearby restaurants close to the Volunteer House or enjoy a meal by the beach. If you prefer to cook for yourself, small shops in Paje sell basic groceries and supplies.

    Meals are served on a rotating weekly menu to offer a good variety of dishes. Dinner portions are shared equally to ensure everyone is accommodated. Filtered drinking water is available at the Volunteer House, and bottled water can also be purchased locally.

    The team will make reasonable efforts to accommodate dietary requirements; however, this cannot always be guaranteed. Volunteers with specific needs may need to purchase additional ingredients at their own cost. Please make sure to note any dietary requirements in your application and inform the local team again upon arrival. For more serious dietary concerns, it is recommended to seek clarification before confirming participation.
