🌍 About the Program

The Zanzibar Medical Project offers a unique opportunity to volunteer in one of the hospitals in Stone Town, on this beautiful, historic island off the coast of Tanzania. Known for its laid-back tropical vibe, Zanzibar is a place rich in culture and challenges alike.

Like much of Tanzania, Zanzibar faces significant healthcare struggles despite being semi-autonomous. With a predominantly Muslim population (about 95%), issues such as early arranged marriages, teen pregnancies, HIV/AIDS stigma, and women’s health remain sensitive and complex topics. Hospitals often work with outdated equipment and limited resources, making every bit of help crucial.

As a volunteer, your role will be to support the medical staff in various departments. Depending on your experience—whether you’re a student, nurse, or doctor—your responsibilities will vary. Initially, you will work under close supervision, gradually earning trust and independence by demonstrating your skills and commitment. Many volunteers have taken on challenging tasks such as assisting in deliveries, taking blood samples, dressing wounds, and even helping with minor surgeries—experiences rarely available in their home countries.

Healthcare facilities in Zanzibar face ongoing challenges: limited and aging equipment, medication shortages, undertrained staff, and high patient volumes. Volunteers play a vital role in improving care quality and easing staff workloads. The hospitals aim to provide the best free medical care possible under these difficult circumstances.

The project collaborates with both government hospitals and smaller private clinics serving local communities. Qualified medical professionals wishing to practice must be certified by the Zanzibar Medical Council—a process that takes about two weeks and requires additional fees and documentation. Once certified, volunteers can stay for up to a year and contribute their specialized skills, including general practice, dentistry, and pharmacy work.

👩‍⚕️ Your Role

You will assist local healthcare teams with a variety of tasks, from HIV/AIDS testing and vaccinations to general ward care. Each day brings new challenges and learning opportunities, requiring adaptability and respect for local customs and procedures.

🧑‍🏫 Guidance and Supervision

Your experience will be supported by our dedicated local coordinator, who will help with your arrival, accommodation, and orientation. Hospital supervisors and staff will provide daily guidance and oversee your activities. Communication, patience, and respect for cultural differences are key to building trust and making your volunteer experience successful.

💵 Additional Costs

A compulsory weekly donation of $100 (paid in cash upon arrival) helps support the hospitals’ ability to supervise volunteers and coordinate work. This fee is not included in your program costs.

🎁 Project Needs

Donations of medical supplies are deeply appreciated but must meet strict criteria: safe, current models with a minimum one-year shelf life and economical running costs. Please consult your booking agent for a full list of needed supplies.

🏠 Accommodation & Community

You’ll likely volunteer alongside both local and international volunteers from all over the world, sharing accommodation that houses up to 16 volunteers.

🛡️ Safety and Cultural Respect

Zanzibar is a safe and welcoming destination but requires cultural sensitivity:

  • Dress modestly outside beach areas, especially for women.

  • Avoid collecting shells or buying them, as this harms coral reefs.

  • Always ask permission before photographing people.

  • Non-Muslims should not enter mosques.

  • Be cautious with personal belongings and aware of your surroundings.

  • Beware of overly friendly locals offering free or cheap services—they often expect something in return.

💰 Transparency: Where Does Your Money Go?

Your fees cover accommodation, staff salaries, transport, and a direct donation to the hospitals to help them stay operational. This funding supports local employment, including your coordinators, and helps the project make a lasting impact.

🌙 Volunteering During Ramadan

Ramadan is a sacred month for Muslims involving fasting from dawn to dusk, prayer, and reflection. Volunteers are not required to fast but should respect local customs by avoiding eating, drinking, or smoking in public during fasting hours and dressing modestly. Greeting locals with “Ramadan Kareem” and understanding the basics of Ramadan etiquette will enrich your experience.

Please note that a police clearance certificate is mandatory to volunteer in the Zanzibar medical project.

Communication is key! Be open and respectful if tasks differ from your expectations or qualifications. Discuss your concerns with your supervisor and look for ways to adjust your role without making demands — remember, you are here to support the team.

Weekends are your time off to explore Zanzibar or, if you choose, to assist with the mobile clinic.

Afhaal aan de luchthaven op Zanzibar International Airport

You will be collected from the airport or the harbor of Stone Town.

As Zanzibar is semi-autonomous and part of Tanzania you can fly straight to Zanzibar (over Nairobi, Kilimanjaro, or other international airports) or fly to Dar es Salaam. If you fly to Dar es Salaam you need to take a cab to the ferry to Zanzibar, where you can buy a ticket for the relaxing 40-minute boat ride to the island. It is very important that we know of your travel arrangements so we can be ready for you to arrive. 

Arriving at the project: Upon arrival at either the airport or ferry terminus, you will be welcomed by the Khaya coordinator who will show you around and take you to your accommodation. After freshening up and getting to know the staff, you will be given an introduction and go through some rules and regulations. Your coordinator will give you a short lesson in Swahili to help you get the basics and show you how to use the daladalas (local taxis). Together with one of the coordinators you will be given a free Stone Town tour in the first week to explore the narrow alleys of this beautiful and atmospheric town. 

Accomodatie

Volunteers will stay in shared accommodation in the area of Chukwani, which is 10-15 minutes away from the centre of Stone Town, the main city on Zanzibar.

The house is fully furnished up to Zanzibari standards, and provides you with a comfortable home for the duration of your stay on Zanzibar. You will stay together with other international volunteers creating lots of opportunities to enjoy the island together.

This is a typical Zanzibar family home. Also on the property is a local bar (tavern) which allows for a nice cold drink with some locals after a long day of volunteering. During the busier months in June & July our volunteers are also hosted at a larger volunteer’s house which can accommodate up to 18 volunteers. The volunteer house has 3 large bedrooms, 2 bathrooms and large kitchen, as well as dining room and lounge area. The house has a garden with a secure wall and gate to make sure you are as safe as possible. Bedding is provided in the form of sheets but bring a towel for personal use. Mosquito nets are not provided so you can bring one if you choose to. If you don’t have enough space in your suitcase: it is cheap to buy things locally. The accommodation can be basic, standards are very different on Zanzibar then back home most probably, and showers do not have hot water, but the weather is hot enough to shower cold! Within easy reach you will find the main road with food stalls to buy fresh juices, Mandazi (local doughnuts) and any other basic needs. Within 20 minutes’ walk you can find 2 popular bars with fast internet and supermarket with some imported (and expensive) goodies to treat yourself every now and then. 

Eten & Drank

🧑‍🍳 Meals
Three basic meals per day are provided by either your host family or the cook at the volunteer house. Meals are based on local ingredients and include a mix of meat, chicken, seafood, and vegetarian dishes, along with local favorites like chips (fries) and pancakes.

🍳 Breakfast
Typically consists of tea, coffee, toast, jam, peanut butter, and seasonal fruit.

🍛 Lunch & Dinner
Enjoy freshly cooked meals after work at your own preferred time. These are hearty and nourishing, giving you the energy to fully experience your volunteering day.

💧 Drinking Water
Safe boiled drinking water is available in the house. If you prefer bottled mineral water, you can purchase it locally at approximately TSh 800 for 1.5 liters (around 0.35 USD — note that exchange rates may vary).

If you crave some extra comfort food or snacks, you're welcome to buy luxury items at your own expense from local shops or markets.
